The main core values of the Connected Senior Care Advantage model are:

  • PERSONAL PHYSICIAN: Each patient has an ongoing relationship with a personal physician who offer continuous and comprehensive care.
  • PHYSICIAN-DIRECTED MEDICAL CARE: The personal physician leads a team of individuals who collectively follow the ongoing care of each patient.
  • COMPREHENSIVE APPROACH: The personal physician provides for all the patient's health care needs, reminding the patient about needed services or arranging for care from other health care professionals. This includes care for all stages of life including acute care, chronic care, preventive services and end-of-life care.
  • COORDINATED CARE: Your Connected Senior Care Advantage team coordinates the care patients need such as specialty care, chronic care management, hospitalizations, and care discharge.

Medicare Advantage plans are typically run and managed by the Health Plan that offers it. A member can use doctors in the network, but the Health Plan approves and covers the care the member receives. The Connected Senior Care Advantage model puts the patient and their physician at the center of the patient’s care. In our agreements with health plans, we shift the services traditionally performed by the health plan into the hands of our primary care physicians. The Connected Senior Care Advantage health care team then works directly with the patient to make healthcare decisions and provide education to help keep our patients healthy and avoid unnecessary procedures, hospital stays and medications.

Finding the right Medicare plan requires careful consideration. Austin Regional Clinic, Premier Family Physicians, Capital Medical Clinic, and South Austin Medical Clinic offer a Medicare Information phone line staffed by licensed insurance agents made available by Empower Brokerage.

Empower Brokerage agents represent all accepted Medicare plans (including Medicare Advantage, Medicare Supplements and Medicare Prescription plans) by these physician offices and always make sure patients receive unbiased information on those plans.

If You Are Soon Turning 65

For patients who are turning 65, there is a window of opportunity where you can select your Medicare coverage. First-¬time Medicare beneficiaries have what is called an Initial Enrollment Period. This period starts three months prior to your 65th birthday, includes your birth month, and extends three months after your birth month. It’s important to enroll during this time.

If You Are Over 65

Patients who have previously turned 65 and opted for coverage through traditional Medicare, can select Medicare Advantage coverage every year during the Medicare Annual Election Period, which runs from October 15 – December 7th.
